Your Checklist for Safer Hiring

We’ve created a checklist for each stage of a driver’s application and employment.

1. Pre-Hire Screening

Purpose: Ensure the driver is qualified, safe, and legally eligible to operate before hire.

  • Driver’s Application for Employment – signed and complete per §391.21
  • Valid CDL Copy – confirm class, endorsements, and expiration date
  • Motor Vehicle Record (MVR) – last 3 years from every state where licensed
  • PSP Report (Pre-Employment Screening Program) – crash and inspection history
  • Employment Verification – previous 3 years Verifiable (plus 7 for CDL safety-sensitive)
  • Drug & Alcohol History Check – from Clearinghouse
  • Criminal Background Check – per company policy
  • Social Security / Identity Verification
  • Road Test or Road Test Certificate – §391.31 or valid equivalent
  • Medical Examiner’s Certificate – valid and on FMCSA registry

2. Compliance File Setup (Driver Qualification File – DQF)

Purpose: Establish and maintain compliance documentation.

  • DQF created and stored (electronic or paper)
  • Copy of driver application and employment history
  • Copy of driver’s current CDL
  • Medical Examiner’s Certificate (and any waivers) Or after 6/23/2025 an MVR
  • Road test certificate or equivalent proof
  • Annual MVR (for renewal)
  • Annual review of MVR
  • Confirm English Language Proficiency (§391.11)
  • Record of previous employer inquiries and responses
  • Training certifications (ELDT, HAZMAT, etc., if applicable)

3. Drug & Alcohol Testing Program

Purpose: Maintain DOT compliance under 49 CFR Part 382.

  • Pre-employment drug test (negative result on file)
  • Enrollment in random testing pool
  • Supervisor training on reasonable suspicion (2 hours required)
  • Policy acknowledgment signed by driver
  • Access to SAP (Substance Abuse Professional) procedures, if ever needed

4. Orientation & Safety Onboarding

Purpose: Train the driver on company procedures, safety, and compliance systems.

  • Orientation session completed and signed
  • Review of company policies & driver handbook
  • Hours of Service (HOS) / ELD training
  • Vehicle inspection and defect reporting (DVIR) training
  • Accident reporting procedures
  • PPE and site-specific safety protocols
  • Fatigue management & distracted driving policies
  • Signature acknowledgment of policies and procedures

5. Post-Hire Monitoring & Continuous Vetting

Purpose: Ongoing compliance, safety tracking, and performance review.

  • Annual MVR review & violation certification
  • Annual medical card update
  • Random drug/alcohol testing participation
  • Periodic performance reviews or ride-alongs
  • Safety meeting attendance records
  • Incident / accident reports filed and reviewed
  • Continuous monitoring (MVR alerts, CSA scoring, etc.)
